When Voyager chases the Equinox, both vessels travel at warp; however, Voyager's warp nacelles appear in the lower impulse position, not in the upward warp position.
When Ransom is attempting to convince Seven to give him the codes, the red glow on her face shifts position between shots.
Noah states there are several deposits of deuterium in the planet surface. Captain Ransom orders him to take an away team and try to localize the ore. Deuterium does not exist as an ore, or even a metal (except where it is highly pressurized). It is an isotope of hydrogen and would usually be found as a gas. On Earth it is found in about 1 out of 6420 normal hydrogen atoms.
If this ratio holds for interstellar space, it would be much more abundant in space, where the interstellar medium contains on average one hydrogen atom per cubic meter.
In Equinox (1999), Captain Janeway states that a Nova class starship has a max speed of warp 8, yet here she states that without the enhanced warp drive, the Equinox is only capable of a maximum of warp 6. She is likely referring to Equinox' capabilities in its current condition.
When Janeway questions Lessing about the whereabouts of Captain Ransom, she leaves the cargo bay and begins to tap the panel on the wall outside to vent the air. You can see the panel move as she taps it.
Janeway is willing to extract information from Noah Lessing by inhumane methods if need be. Chakotay, though disgusted, cannot think of a saner alternative. Both seem to have forgotten that they have a number of mind readers in their crew, who have been helpful for this purpose in the past. Lieutenant Commander Tuvok is the most obvious example.
When captive on the Equinox, The Doctor's ethical subroutines are erased. Immediately upon deletion, he follows any order given to him by his enemy. In reality, the absence of those routines would not cause him to switch sides, only be more unethical in his options to follow whatever course he would choose.
When the Equinox EMH hit the mobile emitter, it deactivated The Doctor. He then (apparently) transferred The Doctor to Equinox and took the mobile emitter as his own. Ethical subroutines gone, it is much more likely he would have just deleted him. As well, the ethical subroutines missing would have transferred as such into the mobile emitter. Once he went to rescue his Equinox crew and deactivated the mobile emitter during the phaser fight, those missing subroutines would have transferred to Voyager as such. The ethical subroutines of The Doctor, now on the Equinox, were at first intact but later deleted as well. This now left both Voyager and Equinox EMHs without ethical subroutines. When the Equinox captain transports his mutinous crew to Voyager, he also (apparently) transfers The Doctor back to Voyager's Sickbay. With the transfer, The Doctor instantly regains his ethical subroutines - which is impossible as there were none on Voyager or Equinox. Even if the captain instantly restored his subroutines on the Equinox before transferring him, those subroutines would not have followed him to Voyager, thus, again making that switch impossible.
